Surgical practice & ASC software built around the procedure
Surgical practice and ASC software is the clinical and business system that carries a patient from the decision to operate through pre-op clearance, the case itself, discharge and post-op follow-up. It is for surgeons, surgical groups and ambulatory surgery centers that need case scheduling, operative documentation, implant tracking and global-period billing in one place. Woltrio designs and builds these systems around your workflow and connects them to the EHR, PACS and billing tools you already run.
Five surgical specialties, one procedure-centered workflow
These specialties share the same backbone: a case that is planned, cleared, performed, billed and followed up. What differs is the imaging, the hardware, the pre-surgical program and the facility rules around it.
General Surgery
General surgery mixes clinic consults, elective cases and add-on urgent cases, so the system has to follow a case from the decision to operate onward. Pre-op workups and clearances (labs, ECG, anesthesia questions, cardiology or primary-care sign-off) become checklists that hold the booking until they are complete. Operative note templates capture procedure, findings, specimens, estimated blood loss and drains. Billing follows US Medicare global surgery rules: 0-, 10- and 90-day global periods, where the 90-day package also covers the day before surgery, plus modifiers 24, 25, 57, 58, 78 and 79 for services inside those periods.

Orthopedics
Orthopedic care depends on imaging and hardware. The chart should open X-ray, CT and MRI studies from your PACS through a DICOM viewer, next to the op note and the implant log. Implant tracking records each device's UDI (in the US, the FDA unique device identifier, with a device identifier plus production identifiers such as lot, serial number and expiration date) so a recall can be matched to patients. Surgeon preference cards drive trays and supply picks for each case. Patient-reported outcome measures for pain and joint function are collected before surgery and at set intervals after it.

Neurosurgery
Neurosurgical cases combine detailed planning with long follow-up. The software brings MRI and CT studies from PACS into the pre-op review and records the planned approach, spinal levels, laterality and any navigation or neuromonitoring needs on the case request. The device record links spinal hardware, shunts or stimulators to the patient by UDI. Op note templates cover levels, instrumentation and intraoperative findings. After surgery, the system schedules imaging and clinic visits across months or years and collects neurological scores and patient-reported outcomes, so every visit can be compared with the pre-op baseline.

Bariatric Surgery
Bariatric surgery starts months before the operation. The system tracks the pre-surgical program as milestones: nutrition counseling sessions, psychological evaluation and clearance, medical workup, education classes and the insurer's pre-authorization. For US Medicare patients, NCD 100.1 ties coverage to a BMI of 35 or higher, at least one obesity-related comorbidity and documented, previously unsuccessful medical treatment for obesity, so that evidence should sit in one place before the request goes out. Commercial payers set their own criteria. After surgery, the program shifts to long-term follow-up: weight trends, labs, supplement adherence and nutrition visits.

Ambulatory Surgery Centers (ASC)
An ASC runs as a facility business as well as a clinical one. Case costing ties supplies, implants, staff time and OR minutes to each case. Anesthesia records capture the pre-anesthesia evaluation, intraoperative events and recovery. In the US, Medicare-certified ASCs must meet Conditions for Coverage (42 CFR 416.52): pre- and post-surgical assessments, written discharge instructions, a discharge order signed by the operating physician and discharge with a responsible adult unless the physician exempts the patient. Medicare-paid ASCs also report to the CMS ASC Quality Reporting (ASCQR) Program, so measure data should be captured during the case.

Core modules of surgical and ASC software
Six modules cover the path of a surgical case. We build the ones you need and connect the rest to systems you already use.
OR and case scheduling
Case requests with procedure, surgeon, estimated duration, equipment and anesthesia type. Block time per surgeon or group, release rules for unused blocks, and room and staff views that show conflicts before the day of surgery.
Learn morePre-op workup and clearances
Checklists driven by procedure and patient risk: labs, imaging, ECG, medication holds, anesthesia questionnaire and specialist clearances. Missing items are flagged to the coordinator, and the case stays on hold until the required sign-offs are in the chart.
Learn moreOperative and anesthesia documentation
Structured op note templates per procedure with findings, specimens, implants and estimated blood loss. Anesthesia records cover the pre-anesthesia evaluation, intraoperative events and recovery, and the data can be exchanged as FHIR Procedure resources.
Learn morePreference cards, implants and supplies
Surgeon preference cards that drive picking lists and trays, with changes tracked over time. Implant and supply use is scanned at the point of care, including UDI where the label carries one, and feeds both the chart and case costing.
Surgical billing and global periods
Charge capture from the op note, global period tracking for 0-, 10- and 90-day procedures, modifier prompts for visits and returns to the OR inside a global period, and pre-authorization status per case before the booking is confirmed.
Learn morePost-op follow-up and outcomes
Follow-up schedules that span the global period and beyond, patient-reported outcome measures sent at set intervals, complication and readmission logging, and dashboards that compare results by procedure, surgeon or implant.
Learn moreCustom vs. off-the-shelf
Off-the-shelf or custom for surgery and ASCs?
Established surgical EHRs and ASC management systems cover common workflows well, and for many practices they are the right choice. Custom software makes sense when your program, facility mix or reporting needs do not fit their templates, or when you need a layer that connects several systems you already own.
Speed to go live
Off-the-Shelf
Usually faster. Standard modules can be configured and live in a short time.
Custom-Built with Woltrio
Takes longer. We phase the build so the first module is in use early.
Pre-surgical programs
Off-the-Shelf
Standard pre-op checklists; multi-month programs like bariatric pathways may need workarounds.
Custom-Built with Woltrio
Milestones, clearances and pre-authorization steps modeled exactly as your program runs them.
Preference cards and case costing
Off-the-Shelf
Often included, with the vendor's cost model and fields.
Custom-Built with Woltrio
Cost model and fields match your supply chain, implant contracts and reporting.
Integrations
Cost over time
Off-the-Shelf
Predictable subscription fees that grow with users, sites or modules.
Custom-Built with Woltrio
Higher upfront investment; you own the code and control future changes.
If an existing product fits, we will tell you; if it does not, we will scope what custom work is worth doing.
What every build includes
These foundations come with every surgical or ASC system we build.
Role-based access for surgeons, anesthesia, nursing, schedulers and billing staff, audit logs on every chart view and change, and encryption in transit and at rest. Software is designed with HIPAA safeguards, and our own information security management is ISO/IEC 27001:2022 certified.
How we approach surgical software
We start from the case lifecycle, not from a feature list, and keep clinicians involved through the whole build.
Case lifecycle first
We map each step from referral to final follow-up with your surgeons, coordinators and billing team before writing code, so the software follows how cases move through your practice or center.
Rules checked against sources
Billing and facility rules such as global periods, ASC Conditions for Coverage and ASCQR requirements are checked against current CMS and FDA publications, and we flag where state law or payer contracts may add requirements.
Phased delivery
We deliver in phases, often starting with scheduling or pre-op tracking, so your team uses real software early and feedback shapes the next module instead of a single large launch.
How a build works
- 01
Discovery
We interview surgeons, schedulers, anesthesia, nursing and billing staff, map your case lifecycle and list the systems the new software must connect to.
- 02
Scope and design
We define modules, data model and integrations, design key screens such as the case request and op note, and agree on a phased plan and estimate.
- 03
Build and integrate
We build in short cycles with regular demos, connect to your EHR, PACS and billing systems, and test with realistic case scenarios.
- 04
Validate and launch
Your clinical and billing leads test workflows end to end, we migrate existing data where needed, train staff and go live in a controlled rollout.
- 05
Support and improve
After launch we monitor, fix issues, update rules when CMS or payer requirements change, and add modules as your practice or center grows.
