HomeHealth TechHome Health, Hospice & Long-Term Care Software
Home health, hospice and long-term care software
Home health and hospice software is the clinical and operational system that agencies and facilities use to assess patients, schedule and verify visits, chart care and bill for services delivered in the home or in a residential setting. It serves home health agencies, hospices, skilled nursing facilities and assisted living communities. Woltrio designs and builds custom platforms, offline-capable mobile charting apps and integrations for these teams.
Care outside the clinic, four specialties
These teams deliver care in patients' homes or in residential facilities, where staff work in the field or across shifts and each setting has its own assessment instrument. They share needs for mobile charting, scheduling, medication records and family communication.
Home Health
Home health software centers on the OASIS assessment. In the US, CMS requires OASIS-E2 (effective April 1, 2026) at time points such as start of care, recertification and discharge, and OASIS items feed the PDGM case-mix grouping that sets Medicare payment for each 30-day period. We build point-of-care OASIS forms with consistency checks, plan-of-care and physician order tracking across 60-day certification periods, visit scheduling with route planning, and electronic visit verification (EVV) that captures the six data points the 21st Century Cures Act requires for Medicaid home health services.
OASIS-E2 · PDGM periods · EVV capture · Route planning
Hospice
Hospice software follows the benefit, not just the visit. The US Medicare hospice benefit runs in two 90-day periods followed by an unlimited number of 60-day periods, and recertification from the third period onward needs a documented face-to-face encounter. Under the Conditions of Participation, the RN initial assessment is due within 48 hours of election, and the interdisciplinary group (IDG) completes the comprehensive assessment within 5 calendar days and updates it at least every 15 days. We build election and benefit-period tracking, IDG meeting workflows, HOPE data collection for the Hospice Quality Reporting Program, and bereavement follow-up.

Skilled Nursing & Long-Term Care
Nursing home software revolves around the MDS 3.0 resident assessment. US facilities complete a comprehensive assessment within 14 days of admission, quarterly reviews, an annual assessment and significant-change assessments, and MDS data drives Medicare Part A classification under PDPM. Day to day, nurses need an eMAR for medication passes, ADL and vitals charting, a resident census with admissions, transfers and discharges, wound and fall documentation, and incident reports routed to the right reviewer. We build these modules or connect them to your existing pharmacy, lab and billing systems.

Assisted & Senior Living
In the US, assisted living is regulated by each state rather than certified by CMS, so documentation rules differ by state and by level of care. Software here is lighter on clinical coding and heavier on daily operations: move-in assessments, resident service plans, medication assistance records, care-level billing, activity calendars and staff task lists by shift. Families expect regular updates, so a family portal with secure messages, visit booking and approved care notes is often central. We build configurable forms and rules so you can adapt to your state's requirements without new code.

Core modules of home and residential care software
Most platforms in this space combine the same building blocks. We build each one to fit your service lines and connect it to the systems you already run.
Assessments and clinical charting
Structured OASIS, HOPE and MDS forms with skip logic, required-item checks and version control, so a new item set can be rolled out on the date CMS sets. Visit notes, vitals, wound photos and care plan goals sit in one chart per patient or resident.
Learn moreScheduling, routing and EVV
Visit scheduling by discipline, frequency orders and staff credentials, with route planning that groups visits by area. Mobile check-in and check-out record the EVV data points, and visits can be exported to your state's EVV aggregator where your state's model requires it.
Learn moreOffline mobile charting
Phone and tablet apps that keep working in basements, on rural roads and in dead zones. Notes, signatures and assessments are stored encrypted on the device and sync when a connection returns, with conflict handling so two clinicians never overwrite each other's work.
Learn moreCare plans and IDG workflows
Interdisciplinary care plans with goals, interventions and responsible disciplines. For hospice, IDG meeting agendas list patients due for review, capture each discipline's input and record plan changes with sign-off, so the review history is easy to show during a survey.
eMAR and medication management
Electronic medication administration records for scheduled and as-needed doses, with pass lists by unit or route, barcode scanning where you use it, refusals and holds, and controlled-substance counts. Order changes can flow in from your pharmacy system instead of being retyped.
Billing and claims
Billing tied to the clinical record: home health periods, hospice levels of care, SNF stays and private-pay assisted living rates. Pre-billing checks flag missing visits, orders or signatures before a claim goes out, and remittances post back to the patient account.
Learn moreCustom vs. off-the-shelf
When to buy and when to build
Established platforms such as WellSky, PointClickCare and MatrixCare cover the standard workflows of home health, hospice and long-term care, and for many organizations they are the right choice. Custom software makes sense when you run several service lines together, need workflows a package does not support, or want to build a product of your own. Many teams do both: keep a core system and build custom apps and integrations around it.
Assessment updates (OASIS, HOPE, MDS)
Off-the-Shelf
The vendor ships new item sets on its own schedule, usually tested across many customers.
Custom-Built with Woltrio
You control the rollout and can add your own validation rules, but you must plan and fund each CMS update.
EVV and state aggregators
Off-the-Shelf
Many states are supported out of the box.
Custom-Built with Woltrio
We build the capture flow and the export format your state's aggregator expects, including less common programs.
Mixed service lines
Off-the-Shelf
Each product is usually strongest in one setting; combining several may mean separate systems.
Custom-Built with Woltrio
One data model for home health, hospice, private duty and residential care, with shared patients and staff.
Integrations
Mobile and field workflow
Off-the-Shelf
A mature app with a fixed workflow you adapt to.
Custom-Built with Woltrio
Screens shaped around your visit types, with offline behavior and route logic designed for your territory.
If a package fits your workflows, we will tell you, and help you integrate it instead of rebuilding it.
Built for care in the field
The platform capabilities that matter when staff work away from a desk.
Designed with HIPAA safeguards: role-based access by discipline and location, encrypted data on devices and servers, remote wipe for lost phones and full audit logs. Woltrio runs an ISO/IEC 27001:2022 certified information security management system.
How we approach these projects
Our method is built around the rules and realities of care delivered outside the clinic.
Regulation mapped before design
We start by listing the assessment instruments, payment rules, EVV requirements and state licensing rules that apply to your service lines, and turn them into acceptance criteria. Every form and workflow traces back to a requirement your team has confirmed.
Designed with field staff
We test prototypes with the nurses, aides and schedulers who will use them, on the devices they carry and in the conditions they work in. Fewer taps per visit and reliable offline behavior come first.
Integrate before you replace
Where an existing EHR or billing system works, we build around it with interfaces and companion apps. Where data must move, we plan the migration in stages with validation at each step, so operations continue during the change.
From first call to go-live
- 01
Discovery
We review your service lines, payers, states and current systems, and map the assessments, EVV rules and billing requirements that apply to each.
- 02
Workflow design
We map a full episode or stay, from referral and intake to discharge, and turn it into screens and data models that clinicians review before any build starts.
- 03
Iterative build
We build in short cycles, starting with the riskiest parts such as offline sync or assessment validation, and demo working software to your team every sprint.
- 04
Testing and migration
Field staff test on real devices and routes. We migrate patient, resident and staff data in stages and reconcile it against your existing system before cut-over.
- 05
Launch and support
We go live by branch or facility, train staff, monitor interfaces and plan updates for new CMS item sets and state rule changes.
