Rehab and therapy EMR software, built around the plan of care

A rehab therapy EMR is an electronic medical record built for outpatient physical therapy, occupational therapy, speech-language pathology, chiropractic and sports medicine practices, where care runs on a plan of care, timed treatment minutes and functional outcomes. Woltrio designs and builds custom therapy EMRs and add-on modules for practices, multi-site groups and software companies that need more than an off-the-shelf system offers.

Five therapy specialties, one documentation model

Physical, occupational and speech therapy, chiropractic and sports medicine all treat patients over a series of visits against measurable functional goals. They share plan-of-care documentation, visit- and minute-based billing, outcome measures and home programs, so one well-designed EMR can serve all five.

Physical Therapy

Physical therapy documentation centers on the initial evaluation, a plan of care with measurable goals, daily notes and progress reports. For US Medicare patients, the system tracks physician or NPP certification of the plan within 30 days of the first treatment, recertification at least every 90 days, and a progress report at least once every 10 treatment days. Flowsheets log minutes for timed codes such as CPT 97110 and 97140 so the 8-minute rule can calculate units, add the GP modifier, and flag CQ when a physical therapist assistant furnished part of the service.

Occupational Therapy

Occupational therapy notes focus on activities of daily living, fine motor skills, cognition and the home or work environment. A custom EMR can hold ADL and IADL assessments, splint and orthosis fabrication records, adaptive equipment recommendations and home safety evaluations next to the plan of care. Claims carry the GO modifier, plus CO when an occupational therapy assistant furnished part of a service. Under Medicare, OT has its own KX modifier threshold, separate from physical therapy and speech-language pathology combined, so the system keeps a per-patient running total by discipline and prompts staff to confirm medical necessity.

Speech-Language Pathology

Speech-language pathology practices document articulation, language, voice, fluency, cognition and swallowing, often with standardized test scores and instrumental swallow study results. Many SLPs also work in schools, where goals are written into an Individualized Education Program (IEP) and sessions are logged against the IEP schedule, or deliver teletherapy that needs video, shared materials and remote consent. Medicare claims carry the GN modifier and count toward the same KX threshold as physical therapy. A useful EMR keeps clinic, school and telepractice caseloads in one record while applying the documentation rules of each setting.

Chiropractic

Chiropractic visits are short and frequent, so SOAP notes need templates a clinician can finish in the treatment room. US Medicare covers chiropractors only for manual manipulation of the spine to correct a subluxation, billed with CPT 98940 to 98942. A subluxation shown by physical exam needs two of the four PART findings (pain, asymmetry, range-of-motion abnormality, tissue or tone change), one of them asymmetry or range of motion. The AT modifier marks active or corrective treatment and must not be used for maintenance care. Personal-injury and workers' compensation cases add attorney correspondence, state-specific forms and narrative reports.

Sports Medicine

Sports medicine clinics combine injury care, rehabilitation and athlete performance, often for teams, schools and clubs. The record needs pre-participation physicals, injury logs by body region and mechanism, links to imaging, strength and range-of-motion testing, and graded return-to-play protocols. For concussion, CDC HEADS UP guidance describes a six-step progression in which each step typically lasts at least 24 hours and the athlete moves up only without new symptoms, under a healthcare provider's supervision. A custom system can track each step, record clearance, and share limited status updates with athletic trainers and coaches when consent allows.

Core modules of a therapy EMR

These six modules carry most of the daily work in an outpatient rehab practice. We build them as a full system or as modules that plug into the software you already run.

Plan of care and certification tracking

Evaluations generate a plan of care with goals, frequency and duration. The system sends it for physician or NPP signature, tracks certification and recertification dates, and warns before a plan lapses or a progress report falls due.

Daily notes and flowsheets

Discipline-specific templates for PT, OT, SLP and chiropractic SOAP notes, with exercise flowsheets that carry sets, reps, load and minutes forward from the last visit. Clinicians document at the point of care instead of after hours.

Learn more

Timed-code billing

Treatment minutes from the note drive billing: units per the 8-minute rule for Medicare, GP, GO or GN discipline modifiers, CQ and CO for assistants, and KX prompts based on each patient's running therapy total. Claims go out through your clearinghouse.

Learn more

Functional outcome measures

Standardized outcome tools are scored at intake, at progress visits and at discharge, then charted against the goals in the plan of care. Practices can compare outcomes by clinician, diagnosis or payer and use them in progress reports.

Learn more

Home exercise programs

Clinicians build home exercise programs from an exercise library with video, photos and dosage, then send them to the patient's phone or portal. Patients log completion and pain, and the therapist sees adherence before the next visit.

Learn more

Scheduling and authorizations

Recurring appointment series match the frequency in the plan of care. The scheduler counts visits used against payer authorizations and case limits, handles multi-clinician and multi-room booking, and fills late cancellations from a waitlist so treatment frequency stays on plan.

Custom vs. off-the-shelf

When a custom therapy EMR makes sense

Established therapy platforms such as WebPT, Raintree and ChiroTouch are the right choice for many practices: they are ready to use and cover common workflows. Custom development makes sense when you run several disciplines or settings, need workflows these products do not support, or are building your own therapy software product.

Time to go live

Off-the-Shelf

Fast. Configure templates, import patients and start within weeks.

Custom-Built with Woltrio

Longer. A first release takes months, so we phase delivery to put the most-used workflows live first.

Mixed disciplines and settings

Off-the-Shelf

Strong for the discipline it targets; mixing clinic, school, teletherapy and chiropractic can mean several systems.

Custom-Built with Woltrio

One record across PT, OT, SLP, chiropractic and sports medicine, with rules applied per discipline and setting.

Billing rules

Off-the-Shelf

Common Medicare therapy rules are built in and updated by the vendor.

Custom-Built with Woltrio

The same rules, plus your payer contracts, PI and workers' comp workflows, and your own edits. You maintain them with us.

Integrations

Off-the-Shelf

Integrations the vendor already supports, through its marketplace or API.

Custom-Built with Woltrio

Direct links to your clearinghouse, wearables, imaging, school systems or data warehouse through FHIR, HL7 or custom APIs.

Cost and ownership

Off-the-Shelf

Predictable per-provider subscription; the vendor owns the roadmap.

Custom-Built with Woltrio

Higher upfront cost; you own the code and decide what gets built next.

If an off-the-shelf product fits your practice, we will tell you so; if it does not, we will scope what a custom build involves.

Built for everyday clinic use

The groundwork that makes a therapy EMR safe, connected and usable on the clinic floor.

Software designed with HIPAA safeguards: role-based access for therapists, assistants, front desk and billing, audit logs on every chart view, encryption in transit and at rest. Woltrio is ISO/IEC 27001:2022 certified, and we sign a Business Associate Agreement (BAA) with healthcare clients before PHI is shared.

How we approach therapy software

We start from your clinicians' workflow and the payer rules that apply to it, then build only what your practice needs.

Clinician-led discovery

We sit with therapists, assistants, front desk and billing staff to map how an evaluation becomes a plan of care, daily notes and a claim. Templates and screens follow that real sequence rather than a generic form.

Rules from the source

Billing and documentation logic is written from the current CMS manuals and your payer contracts, and each rule is documented so your team can check it. When CMS updates a rule, the change goes into a defined maintenance process.

Modular delivery

You can start with one module, such as home exercise programs or timed-code billing, alongside your current EMR, and extend from there. Each module has its own API, so it can stand alone or join a full system later.

From first call to go-live

  1. 01

    Discovery

We review your disciplines, settings, payers and current software, and map, step by step, the plan-of-care, documentation and billing workflows the system must support.

  2. 02

    Design and prototype

Clinicians test clickable prototypes of evaluation, daily note and flowsheet screens before development starts, so changes happen early, while they are still cheap to make.

  3. 03

    Build in phases

We develop in short iterations, releasing core charting and scheduling first, then billing rules, outcome measures and home exercise programs, with a demo at the end of each iteration.

  4. 04

    Migration and integration

Patient records, open plans of care and authorizations move from your current system, and interfaces to your clearinghouse and referral sources are tested with real scenarios.

  5. 05

    Launch and support

Staff are trained by role, go-live is supported on site or remotely, and we maintain the system as payer rules, CMS updates and your workflows change.

Frequently
Asked Questions

Seeking basic information? Our FAQ section is a ready reckoner with precise answers to the most probable queries.

What is a rehab therapy EMR?

A rehab therapy EMR is an electronic medical record designed for outpatient physical therapy, occupational therapy, speech-language pathology, chiropractic and sports medicine. Unlike a general medical EMR, it is organized around the plan of care: an evaluation sets goals, frequency and duration, daily notes record timed treatment minutes, progress reports measure change, and billing is calculated from the documented minutes. Most also include home exercise programs and functional outcome measures. See how this fits with a broader custom EMR/EHR build.

How does the software apply the 8-minute rule?

The software adds up the minutes documented for each timed CPT code in a visit and converts the total into billable units under the Medicare method. Under that rule a single timed service needs at least 8 minutes to bill one unit, and 23 to 37 minutes bills two units. When several timed services are performed, units are based on total timed minutes, with more units going to the services that took the most time. Clinicians enter minutes in the note; the system calculates units and shows them before the claim is created.

Can the system track plan of care certification and progress reports?

Yes. For US Medicare patients, the system tracks that the physician or NPP certifies the plan of care within 30 days of the first treatment, that recertification happens at least every 90 days or when the plan changes significantly, and that a progress report is written at least once every 10 treatment days. Dashboards show plans awaiting signature and reports coming due, so nothing is missed before a claim goes out. Commercial payer rules can be configured separately.

What is the KX modifier and how does the EMR support it?

The KX modifier tells Medicare that outpatient therapy above the annual therapy threshold is medically necessary and supported by documentation. Physical therapy and speech-language pathology share one threshold, and occupational therapy has a separate one; claims above the threshold without KX are denied. The EMR keeps a running total per patient and discipline, alerts staff as the threshold approaches, and requires a documented medical-necessity justification before KX is added. Billing workflows connect to our medical billing and RCM modules.

Can one EMR serve physical therapy, OT, speech therapy and chiropractic?

Yes, if it is designed for it. A shared patient record, scheduler and billing engine can sit under discipline-specific templates, modifiers and rules: GP, GO and GN modifiers for the therapy disciplines, and the AT modifier and PART documentation for Medicare chiropractic care. Each clinician sees the forms for their discipline, while the practice gets one schedule, one ledger and one patient history. Multi-discipline groups are a common reason to choose a custom build over a single-discipline product.

Do you support teletherapy and school-based speech therapy?

Yes. Teletherapy sessions can run inside the EMR with video, screen sharing for therapy materials, remote consent and the same note templates used in clinic. For school-based SLPs, the system can record IEP goals, log sessions against the IEP service schedule and keep school caseloads separate from clinic patients. Payer and state rules for telehealth vary and change, so they are configured rather than hard-coded. Learn more about our telemedicine software.

Can you migrate data from our current therapy EMR?

Yes. We migrate patient demographics, insurance, open plans of care, notes, outcome scores and authorization counts from your current system, depending on what it can export. We map the data, run test migrations, and reconcile counts with your team before cut-over, so treatment and billing continue without a gap. Read more about our approach to healthcare data migration.

Is a custom therapy EMR HIPAA compliant?

We build therapy EMRs designed with HIPAA safeguards and built to support HIPAA compliance: role-based access, audit logging, encryption, backups and secure hosting. Compliance also depends on how your practice operates the system, so we document the controls and support your risk assessment. We sign a Business Associate Agreement (BAA) with healthcare clients before PHI is shared. Details are in our Trust Center and on our HIPAA-ready software page.
