HomeHealth TechEye Care EMR (Ophthalmology & Optometry)
Eye care EMR software that fills from your diagnostic devices
An eye care EMR is an electronic medical record built for ophthalmology and optometry, where exam findings, images and device measurements land in the chart per eye without retyping. Woltrio designs and builds custom eye care EMRs for general and subspecialty ophthalmology practices, optometry groups and optical shops. We cover OCT and biometry integration, cataract and injection workflows, and the optical counter.
Three eye care practices, one shared exam record
Ophthalmologists, retina and glaucoma subspecialists and optometrists examine the same eye with the same devices, and often share patients through referrals and post-op co-management. One record lets measurements, images and prescriptions follow the patient across all three.
General & Surgical Ophthalmology
General ophthalmology needs fast exam templates for visual acuity, IOP, slit lamp and dilated fundus findings, with anatomical drawings per eye. Cataract surgery adds its own chain: biometry and keratometry flow from the device into the IOL calculation, and the selected lens and target refraction carry into the surgical plan, followed by consents, the operative note and post-op visits. In the US, Medicare co-management means billing the surgery with modifier 54 and post-op care with modifier 55, backed by a written transfer agreement and the dates care was relinquished and assumed.

Retina & Glaucoma Subspecialties
Retina clinics run on intravitreal injections: each visit records the eye, drug, dose, lot number and expiry, the OCT that drove the decision and the interval to the next injection. US Medicare claims pair CPT 67028 with the drug's HCPCS code and an RT or LT modifier, and different drugs in each eye on the same day need a documented rationale. Glaucoma care depends on trends: IOP over time, visual field indices from static perimetry and optic nerve OCT, plotted side by side against the target pressure so progression is easy to see.

Optometry & Optical
Optometry combines medical eye care with refraction, contact lens fitting and an optical shop. The record needs manifest and cycloplegic refraction, glasses and contact lens prescriptions, and an optical point-of-sale linked to frames, lens orders and lab status. In the US, the FTC Eyeglass and Contact Lens Rules require releasing the prescription to the patient and, for practices that sell eyewear, keeping a signed confirmation for at least three years. Billing splits between vision plans and medical insurance, since Original Medicare excludes refractions.

Core modules of a custom eye care EMR
Six modules cover the path from check-in and testing to surgery, injections and the optical counter.
Device integration & imaging
OCT, visual field, fundus camera, biometer and autorefractor results flow into the chart. Where devices support DICOM ophthalmic objects, we import images and measurements directly; otherwise we integrate through vendor exports, files or the imaging software already in use. Every result is matched to the right patient, eye and visit.
Learn moreOphthalmic exam templates & drawings
Templates follow how eye exams actually run: history, acuity, pupils, IOP, slit lamp, dilated exam, assessment and plan, all recorded per eye. Clinicians sketch on retina and anterior segment diagrams, and normal findings fill in with one tap so documentation stays quick for technicians and doctors.
Cataract surgery workflow
Biometry and IOL calculation results, the chosen lens and the target refraction move into the surgical plan without retyping. The module tracks consents, lens option selection, surgery scheduling, operative notes and the post-op visit sequence, including the handoff to a co-managing optometrist.
Learn moreInjection & drug lot tracking
Each intravitreal injection is logged with the eye, drug, dose, lot number and expiry. Fixed or treat-and-extend intervals drive recall, and the injection history sits next to OCT findings so the physician sees response and timing in one view. Drug stock can be reconciled against recorded usage.
Refraction, prescriptions & optical
Refraction results become glasses and contact lens prescriptions, released to the patient on paper or digitally, with a recorded confirmation where required. The optical module handles frame and lens inventory, point-of-sale, lab orders with job status, and pickup notifications.
Medical and vision-plan billing
The same visit can be billed to medical insurance or to a vision plan, depending on why the patient came in. Billing rules route each service to the right payer, apply eye laterality modifiers, and keep refraction and materials separate from medical exam charges. US payer specifics are configured per contract.
Learn moreCustom vs. off-the-shelf
Custom eye care EMR or an established platform?
Established eye care platforms such as Nextech, ModMed EMA Ophthalmology and Compulink cover the common ophthalmology and optometry workflows, and for many practices one of them is the right choice. Custom development makes sense when your device mix, subspecialty protocols, optical operation or multi-site setup don't fit a standard product, or when you are building eye care software to sell.
Device integration
Off-the-Shelf
Interfaces for common device models, usually from the vendor's supported list and imaging add-ons.
Custom-Built with Woltrio
Integrations built for the exact devices you run, including older units, using DICOM, file exports or vendor APIs.
Exam templates
Off-the-Shelf
Configurable templates that suit most general ophthalmology and optometry visits.
Custom-Built with Woltrio
Templates and drawings shaped around your subspecialty protocols and how your technicians work up patients.
Surgery and injection tracking
Off-the-Shelf
Standard surgery and injection modules; depth varies by product and edition.
Custom-Built with Woltrio
Lot tracking, treat-and-extend logic and co-management handoffs built around your clinic's rules.
Optical shop
Off-the-Shelf
Often included, or available through a partner optical system.
Custom-Built with Woltrio
Optical POS, inventory and lab ordering joined to the same patient record and prescriptions.
Cost and time to start
Off-the-Shelf
Faster to start, with subscription pricing and vendor-managed updates.
Custom-Built with Woltrio
Higher upfront investment and a build phase; you own the roadmap and the data model.
If a standard product fits your practice, we will say so; if it doesn't, we scope exactly what needs building.
Built for clinical and practice operations
The groundwork every eye care system needs, beyond the exam screen.
Role-based access separates technicians, doctors, opticians and billing staff, with audit logs on every chart view and change. Systems are designed with HIPAA safeguards, and Woltrio is ISO/IEC 27001:2022 certified. We sign a Business Associate Agreement (BAA) with healthcare clients before PHI is shared.
How we approach eye care software
We start with your devices and your clinic flow, then build only what a standard product can't cover.
Device inventory first
Before design starts, we list every device in your clinic with its model, software version and export options. That inventory decides whether DICOM, file import or a vendor interface is used, and it surfaces integration risk early rather than at go-live.
Clinicians in the loop
Exam templates and drawings are reviewed with your ophthalmologists, optometrists and technicians in short cycles. We test them against real visit types, such as a post-op day-one check or an injection visit, before they reach production.
Compliance in the workflow
Prescription release, consent capture, lot tracking and audit trails are built into the workflow rather than added later. Systems are built to support HIPAA compliance and the record-keeping rules that apply in your country.
From discovery to go-live
- 01
Discovery & device audit
We map your visit types, subspecialties, optical operation and billing mix, and list every diagnostic device with its interface options. You get a written scope and integration plan.
- 02
Workflow design
Exam templates, drawings, surgery and injection flows are designed as clickable prototypes and reviewed with clinicians and technicians until they match how your clinic works.
- 03
Build & integrate
We build in short sprints, connecting devices, practice management, labs and clearinghouses as we go. Each sprint ends with a demo on test data.
- 04
Data migration & testing
Charts, images, prescriptions and injection histories move from your current system, followed by clinical, billing and security testing with your staff, so problems surface before go-live.
- 05
Launch & support
We go live by site or by module, train staff on real visit types, and stay on for fixes, device updates and new features.
